Stage 5: References
Use this form to add persons that you would like to use as references.
Your references must be individuals that are not family members, and at least one of them must be a pastor or head elder. References must have known you for more than one year.
Persons chosen as referrals can complete the form online (we prefer this method); indicate this when you add them. You can see the progress of the references below (you won't have access to the contents of the references).
Add a minimum of four (4) references. References that will be submitted manually (via regular mail) will also count to your total. (You can delete any references that you created in error, or those that can't be completed.)
